Family Caregiver Support provides case management and support groups for persons responsible for the care of a loved one. A growing segment of our population is known as the “sandwich generation.” These individuals are responsible for teen/adult children and often grandchildren, as well as their own parents or other elderly family members while maintaining employment and planning for their own retirement.
- Family Caregiver Support offers resources, planning and implementation of care and respite options, long term planning, support groups and guest speakers.
- Home visits are made to assess the care receiver and home environment for disabled modifications.
- Monthly monitoring is achieved through telephone or additional visits. Hospital visits are made as needed.
